Social Services

Our Social Workers primary concern is with your complete well being and recognize that illness causes much concern and anxiety and can increase personal or family problems.  A Social Worker assigned to each floor, who works in close collaboration with your physician and other health care team members, can help you and your family with those worries or problems interfering with your medical care or progress. 

In addition to counseling, the Social Worker can assist you with homecare, rehabilitation, nursing home and various community service referrals to best meet your discharge planning needs. Please contact the Social Work Department at extensions 7214, 7215, or 7216.

The Social Work Department provides our patients with safe and effective hospital discharge plans and is especially sensitive to the diverse cultural needs of the patients we serve.  The Department is committed to educating families regarding the social and psychological problems that result from illness and to maximizing the coping skills of the family unit in order to optimize the outcome of the patient's medical treatment.

The Department's professional social workers provide services to patients of all ages who have been identified with biopsychosocial, medical, financial, and discharge planning needs. Assistance to patients and their significant others include:

  • Nursing home placement
  • Homecare arrangements
  • Crisis counseling
  • Bereavement counseling
  • Urgent services such as:  shelter, food, clothing, equipment
  • Adoption and family planning
